Supplemental Medical

Green Dot Public Schools offers four types of supplemental medical plans, which provide you with cash payments in the event of a significant unexpected medical expense. Be sure to consider your anticipated medical needs for the coming year — for example, a major surgery — when deciding if supplemental coverage is right for you.

2021 – 2022 Aflac Supplemental Medical Plans

You may choose to enroll in any combination of the following Aflac plans:
  • Personal Accident Coverage
  • Hospital Indemnity Plan
  • Personal Cancer Coverage
  • Critical Illness/Injury and Intensive Care Coverage
  • Aflac Short-term Disability Insurance

You pay premiums for these coverages through payroll deductions.

On their own, these plans don’t provide the minimum level of medical coverage needed to meet healthcare reform requirements. Rather, they’re intended to supplement the coverage provided by your medical plan.

Critical Illness/Injury and Intensive Care Coverage

When a serious illness or injury strikes, Aflac’s critical illness/injury and intensive care coverage can provide financial support to help you through a difficult time. It protects against the financial impact of certain illnesses, such as a heart attack, cancer, or stroke.

You receive a lump-sum benefit upon diagnosis of a covered event with additional benefits to be paid for things such as a hospital confinement, intensive care unit confinement, ambulance, transportation, lodging, and therapy. Coverage is portable, so you can take it with you if you leave Green Dot Public Schools.

Benefits are paid:

  • Directly to you, unless assigned to someone else
  • As a lump sum or on a benefit schedule

 

 

Personal Accident Coverage

Accident insurance helps protect you from unexpected financial stress if you or a covered family member has an accident. It supplements your primary medical plan by providing cash benefits in cases of accidental injuries. You can use this money to help pay for medical expenses not paid by your medical plan (such as your deductible or coinsurance) or for anything else (such as everyday living expenses). Coverage is portable, so you can take it with you if you leave Green Dot Public Schools.

Benefits are paid:

  • Directly to you, unless assigned to someone else
  • In addition to any other coverage, like your primary medical plan or an accidental death & dismemberment (AD&D) plan

 

You receive a cash benefit up to a specific amount for:

  • Accidental death
  • Dismemberment
  • Dislocation or fracture
  • Initial hospital confinement
  • Intensive care
  • Ambulance

The actual benefit amounts depend on the type of injuries you have and the medical services you need.

Aflac Short-term Disability Insurance

Short-term disability insurance, available through Aflac, provides a source of monthly income if you are unable to work due to an illness, injury or pregnancy. Coverage is portable, so you can take it with you if you leave Green Dot Public Schools.

Plan features:

  • $500 to $6,000 month benefit in addition to State Disability for CA employees
  • Guaranteed issue coverage – no health questionnaire to answer
  • Rates stay the same every year

Benefits are paid:

  • Directly to you, unless assigned to someone else
  • In addition to any other disability benefits, such as short-term disability coverage provided by Green Dot Public Schools or California State Disability benefits

Personal Cancer Coverage

Personal cancer coverage is available through Aflac to help you and your family better cope financially — and emotionally — if a cancer diagnosis ever occurs. Coverage is portable, so you can take it with you if you leave Green Dot Public Schools.

Coverage is available for:

  • Initial diagnosis
  • Radiation and chemotherapy
  • Hospital confinement
  • Various cancer treatments
  • Surgery and anesthesia
  • Prosthesis
  • Transportation and lodging
  • Anti-nausea drugs
  • Home health and nursing services
  • And more

Benefits are paid:

  • Directly to you, unless assigned to someone else
  • As a lump sum or on a benefit schedule

Hospital Indemnity Plan

A trip to the hospital can be stressful, and so can the bills. Even with a major medical plan, you may still be responsible for copays, deductibles, and other out-of-pocket costs. Aflac’s hospital indemnity plan can help offset your share of the cost associated with a covered sickness or accident.

Use this money to help cover hospital stays, emergency rooms, rehabilitation, and certain extended benefits through optional riders. The plan pays benefits in addition to any other insurance. Coverage is portable, so you can take it with you if you leave Green Dot Public Schools.
